Hospice had her on Bactrim about a month ago, then after a few weeks had her do another course of it. She is not better, delerium, discomfort,not sleeping even with Olanzipine, lorazepam, myrtazapine, melatonin.sleeps a lot in day but is often up for hours at night talking and feeling upsetshe is eating a little better now.
Typically there are meds that are "formulary" that can be prescribed without approval.
If necessary there is always the option to pay for a non covered medication out of pocket.
Also, it's apparent that the melatonin isn't working so I would stop giving it to her. Giving someone too much or at the wrong time can cause more problems than it solves. The delirium (and dementia if she has any) is messing up her normal sleep so not sure a supplement will fix it especially with all the other meds she's on.

Is she getting enough fluids?
When does the nurse practitioner or doctor come to check on her...
Have a heart to heart talk with dr.
Play mom's favorite music, tell stories, make sure dr gives her something to make her comfortable.
Maybe her body just can't fight this UTI as easily as before..
Maybe your hospice has a musician who can play a few songs for your mom.
If mom has a strong faith, you can ask hospice to bring in someone of her faith to talk with her and you.
Flowers.. lighten up her atmosphere with flowers. If she's not on oxygen, light up a couple of candles.
Try to get more fluids down her. Does she like Jello..
my mom graduated out of hospice 3 times.. the 3rd time she went to heaven..
